Please read through the Welcome letter and Checklist of required materials for your intended class.  These documents are required only AFTER an application has been submitted and you have secured a spot in your desired class.

Please submit all forms to: Discovery Parent Child Preschool 1919 Gunston Way.  San Jose, CA 95124 by their respective due dates.

Registration Materials for Two's Class

Set 1: Due June 29

1. First month's (October) tuition -

3. Child's Preadmission & Health History

4. Parent Contract

5. Consent for Medical Treatment & Emergency Transport

6. Personal Rights

7. Notification of Parent's Rights/Caregiver Background Check

Set 2: Due August 5

1. Physician's Report - Child Care Centers

2. Adult TB Test

3. Adult Immunization Record - submit documentation from your physician's office.

4. Credit Card Authorization Form (optional)

8. Adult Immunization Record - submit documentation from your physician's office.  The immunizations required in order for you to work in the classroom are: Influenza, Pertussis, and Measles.
